Fixed shield exhaustion applying when the player stops using the shield during the transition to another dimension

Merged health/hunger effects to allow health/hunger min/max to apply to sleep effects

Hunger should no longer be taken while in creative/spectator

Ability to choose whether the effect needs to pass both the health and hunger bounds or only one
