Added 3 new difficulty options - Nightmare, Insane and Custom
Added more loot options and change old loot tables for each difficulty
Custom difficulty: You can set your own values of HP, damage and loot (there's 10 loot tables so far, I'll add/change them according to your feedback)
Removed Warden's Fragment, Dark Essence and Void Fragment temporarily (they'll get their use in v1.3)
Fixed some compatibility issues with older versions (there should no longer be any issues with versions 1.21 - 1.21.4)
Added support for 1.21.10 and 1.21.11
